Our practice is built on the belief that patients
want to receive the highest quality dental care
available, because it provides them with the
peace of mind that comes from knowing their
mouth is healthy and that they will be able
to keep their teeth for a lifetime.

In order for Dr. Banks to provide patients with
quality care, fees are based on the amount of
care, skill and judgment that go into providing
those services. When fees are determined by
a third party to satisfy the bottom line without
taking into consideration the high standards
of this office, then something must be compromised.
This compromise can manifest itself in such
ways as less time spent with patients, use of
lower quality materials, and inferior laboratory
support.

As a courtesy to our patients who do have insurance,
we will provide a claim form with an itemized
receipt for you to mail to your insurance company.
The insurance company will then reimburse you
directly. In order to do so, please bring in
your patient insurance I.D. card. We will also
need the name of the insured, his/her place
of work, the carrier's social security number,
and birthdate. This information will enable
us to print an accurate and detailed claim form
for you.

Payment is expected in full on the day of each
appointment. If other arrangements are needed,
our financial administrator will be happy to
speak with you prior to your appointment. We
make every effort, when possible, to inform
the patient of the fees for service prior to
their appointment. Our office accepts cash,
personal checks, Visa, and MasterCard.
